To order, simply select the matching plaque size with how many individual plates above. Add any files or any special instructions you may have. Below are the plaque sizes available and matching plates for your reference. We will have one of our graphic designers draw up a proof and send it to you for approval. Once approved, we will put your product into production.  

Individual plates are 7/8" x 2-3/4"
